A 18 year old male,from miryalaguda,who is a student, came to the hospital with chief complaints of low backache 1 week ago,fever since 5 days ,yellowish discolouration of eyes since 3 days ,vomitings (2 episodes) and loose stools(3 episodes)and blood tinged urine yesterday morning.
          
My opinion:
                   The history of present illness  And past history are given briely and is easy to understand.The general examination is given clearly with diagram .The investigations are properly given with the pictures attached.The updated treatment with timeline are given.the provisional diagnosis is mentioned which is acute viral hepatitis.Overall presentation is good but can be elaborated and clearly explained.

Question no.4

A 60yr old female presented to the OPD with chief complaints of pedal edema since 10 days, decreased urine output since 10 days and fever since 10 days.

My opinion:
                  The history for this case is very well presented both the general examination and systemic examination.Especially,the details In the systematic examination are given perfectly according to different systems.Th list of investigations are mentioned very clearly with the diagrams.The investigations are given according to the timeline.There is no confusions in investigations.The diagnosis is mentioned which is acute kidney injury.The treatment is given in which date and time included.Overall,it is very well presented.

This assignment is very helpful in all aspects.I learned how to take the history and how to present a case and how to reach to a probable diagnosis using the information provided.These elogs are very helpful as we are not able to attend the clinical postings in the hospital.I also learnt about a few diseases through the diagnosis.some cases are well presented in an understandable manner.These elogs can be very helpful for our further clinical postings.Even though I don't know much about the subject,I learnt which points are necessary for the diagnosis and which points can be omitted.The elog method is very effective and encouraging to the students during this pandemic as we can learn more.
